Welcome to the Hopewell Healthcare Management mobile application (the "Hopewell HMO App"). By downloading, accessing, or using the App, you agree to be bound by these Terms and Conditions and our Privacy Policy, which is incorporated herein by reference.

1. Acceptance of Terms

These Terms and Conditions constitute a binding agreement between you and Hopewell Healthcare Management Limited ("Hopewell Healthcare", "we", "us", or "our") governing your use of the App. If you do not agree to these Terms and Conditions, you should not use the App.

2. Use of the App

The App is provided to you for your personal, non-commercial use. You agree to use the App in compliance with all applicable laws, rules, and regulations. You are responsible for obtaining and maintaining any equipment or ancillary services needed to connect to, access, or otherwise use the App.

3. Account Registration

When creating an account, you agree to provide accurate and complete information. You are responsible for maintaining the confidentiality of your account credentials and for any activity that occurs under your account. To use certain features of the App, you may be required to register for an account.

4. Eligibility Criteria (Age Restriction)

Individuals aged 60 and above from the date of sign-up are not eligible to enroll in our health plans.

5. Enrollment and Access to Care

a. Effective Date of Coverage: You are eligible to access care almost immediately after sign-up, provided all required information and payment have been received.

b. Initial Access Restrictions: For the first 3 months from the effective date of coverage, access to care is limited to primary services only. This includes routine check-ups, consultations, and preventive care.

c. Full Access: After completing the initial 3-month period, you will have full access to all covered health services, including specialist referrals, hospital care, and other benefits outlined in your chosen health plan.

6. Intellectual Property

The App and its entire contents, features, and functionality (including but not limited to all information, software, text, displays, images, video, and audio, and the design, selection, and arrangement thereof) are owned by Hopewell Healthcare Management Ltd, its licensors, or other providers of such material and are protected by copyright, trademark, patent, trade secret, and other intellectual property or proprietary rights laws.

7. Third-Party Services

The App may contain links or connections to third-party websites or applications. Hopewell Healthcare is not responsible for the availability, content, or privacy practices of those third-party services. Your use of such third-party services is at your own risk and subject to their respective terms and conditions.

8. Disclaimers and Limitations of Liability

The App is provided "as is" and "as available" without warranties of any kind, either express or implied. Hopewell Healthcare does not warrant that the App will be uninterrupted or error-free. To the maximum extent permitted by law, Hopewell Healthcare shall not be liable for any indirect, special, incidental, or consequential damages arising out of or related to your use of the App.

9. Termination

Hopewell Healthcare Management Ltd reserves the right to suspend or terminate your access to the App at any time for any reason, including if we reasonably believe you have violated these Terms and Conditions.

10. Governing Law and Dispute Resolution

These Terms and Conditions shall be governed by and construed in accordance with the laws of the Federal Republic of Nigeria. Any disputes arising out of or related to these Terms and Conditions or the App shall be resolved through binding arbitration.

11. Changes to Terms and Conditions

Hopewell Healthcare reserves the right to modify these Terms and Conditions at any time. We will notify you of any material changes by posting the updated Terms and Conditions on this page.

12. Acknowledgement

By signing up for our health plans, you acknowledge that you have read, understood, and agreed to these terms and conditions. You confirm that you meet the eligibility criteria and understand the access restrictions during the initial 3-month period.

13. Disclaimer

We reserve the right to verify information and adjust or terminate coverage if eligibility criteria are not met or if false information has been provided. Please review these terms carefully before signing up. If you have any questions or concerns, please contact our customer support team. By Signing up and using the Hopewell Healthcare Management App, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.

IMPORTANT NOTES

a. It is your responsibility to provide accurate information during the sign-up process.

b. Failure to disclose material information or provide false information may result in termination of coverage or denial of claims.

c. Our health plans are subject to change, and we reserve the right to modify or update these terms and conditions at any time.
